Support services provided by the Nexus program expand the options available to newly diagnosed or People Living with HIV giving them the benefit and flexibility of receiving HIV-related care in a medical setting of their choosing – a private infectious disease, primary care or specialist practice. Your doctor can refer you to Nexus and based on your eligibility you may enroll in the appropriate support services to supplement your medical treatment and help you achieve and maintain viral load suppression. Interested participants must: Commit to a long-term, abstinence-based recovery program for 6-9 months to include a minimum of 3 months in transitional living. Adhere to our acceptable medications list. Disclose any mental health diagnosis. Bring a 30-day supply of needed/prescribed medications. Be able and willing to walk up to 5 miles per day and work various jobs in the building. Agree to not work outside of the building for money during this time.

They started in 1970 as a community-based and patient-directed organization focused on providing care to people with limited access to the health services they need. They are committed to and must uphold a high level of care, meeting administrative, clinical, and financial standards as part of responsibility as a FQHC. They offer comprehensive care: medical, dental, behavioral health, pharmacy, and at some sites, x-ray services. The majority of their governing board is made up of their patients – at least 51% of board members must be representative of the patients they serve.

Our inpatient rehabilitation hospital uses an interdisciplinary team approach that includes physical, speech and occupational therapists, rehabilitation physicians, rehabilitation nurses, case managers, dietitians and more, combined with our advanced technology and expertise, to help you achieve your goals. You will receive at least three hours of therapy five days per week while under the constant care of registered nurses, many of whom specialize in rehabilitation, and frequent independent private practice physician* visits. Rehabilitation is covered in full or part by most health insurance plans, including Medicare and Medicaid. Our hospital has contracts with most major insurance companies and is willing to negotiate with other companies, as needed. *The hospital provides access to independent physicians.
